Chicago Med begins tonight with Dr. Natalie Manning (Torrey DeVitto) walking into the ED and Dr. Will Halstead (Nick Gehlfuss) having to speak to her about being late to work. Will gives her a case for a child with a bleeding nose, insisting she is fine and nothing is wrong after her TBI, but he tells her it isn’t his decision.

Dr. Ethan Choi (Brian Tee) winks at April Sexton (Yaya DaCosta), trying to be sexy. Dr. Noah Sexton (Roland Buck III) notices everything, joking how it is now burned into his retinas. Noah admits school is intense as Ethan reminds him it is always a body on the table. Dr. Crockett Marcel (Dominic Rains) arrives at takes Noah with him on his rounds, giving Ethan a chance to quiz April about Noah’s new mentor. Meanwhile, Sharon Goodwin (S. Epatha Merkerson) runs into Dr. Daniel Charles (Oliver Platt) and his wife, Caroline “CC” Charles (Paula Newsome), commenting on how happy the two of them are; Daniel leaves CC with Sharon as he deals with a patient in the ED.

Maggie Lockwood (Marlyne Barrett) is at home, when she removes the wrap from her head, revealing she is losing her hair; she places on her wig as she prepares to go to work.

At Chicago Med, Ethan deals with his patient, Carl complaining of abdomen pain. He is from Tennessee, making Ethan want to see their records. He requests several tests, promising to try to be the last doctor he sees in hopes of figuring out his problem. Natalie meets Lucas who plays peek-a-boo with her. His mother explains how he was at the park and his nose started gushing. She admits they all have allergies, explaining Lucas’ symptoms. Natalie feels it is pointing to allergies but since he had a fever, she wants to do blood tests; his parents Janet (Amanda Quaid) and Kyle Harper (Alex Kramer) follow the natural way but do agree.

Maggie tells April she needs her to take over when she goes to her appointment and she won’t be back today. April asks another nurse, who also has no clue what is going on with Maggie but she caught her sleeping in a treatment room the other day and she nearly bit her head off when she asked if she was okay.

Marcel continues to wow Noah with his story of pulling out a 3-foot tapeworm out of a patient. Ethan observes them, making a comment to April, who feels that Ethan may not like him because his style of teaching is different than his. She admits Marcel is a good surgeon as Ethan receives the first results of Carl’s tests. His white cell is high, so it means more tests. Noah overhears Ethan’s case and tells Marcel about it, suggesting it is a case worth looking into.

Dr. Charles is with the CPD who has Eunice Kim (Greta Jung) who has been pacing and ranting; with no sleep in over 2 days. They found nothing sketchy in her apartment so he walks into the room, attempting to speak to her. He sees that she is under an immense amount of pressure, offering to give her something to make her think more clearly, making her realize she doesn’t know where she is but he assures her that she is somewhere safe.

Ethan runs off when he gets a message, Carl who is freaking out over his scan. They are able to get him out of the machine, only for him to vomit all over Ethan, who is just happy Carl made it through the test.

Will’s patient has a very swollen calf, pain and has DVT; a blood clot in his leg. He asks Will to grab him his phone, so they can let him know what is going on. Meanwhile, Marcel and Noah are doing a surgical consult on Carl; infuriating Ethan, who demands to speak to them outside. He confronts Noah on snooping through the ED for surgical cases and Marcel is not a good mentor teaching his students how to poach patients. He tells them to stand down, upon pending results he will notify them if he needs them. Marcel apologizes and blames Noah for proper etiquette.

Natalie asks Doris if there are any results back for Lucas yet. Natalie confronts Maggie, demanding to see more patients as the ED is full but Maggie reminds her that Dr. James Lanik (Nate Santana) was very clear about her having a light caseload only. Maggie excuses herself, telling April she is leaving; confusing Natalie.

Daniel returns to see Eunice who is embarrassed. She confesses it was a blur but she knows it wasn’t herself. She promises that no matter how busy she is, she will get some sleep. She feels everyone is overreacting to a panic attack but he explains she was having a manic episode. She is in complete denial and he tells her if she ignores it, this will become an extremely vicious cycle. She insists she can control this, but he wants to admit her so they can teach her how to manage life with it. She knows the criteria for involuntary commitment and he cannot keep her there. She is persistent about leaving.

Marcel concurs with Ethan, saying Carl needs to have a mass returned from his pancreas. Ethan explains the mass might not be the issue but Marcel pushes that it could have been building over time and he needs surgery. Ethan feels if he performs surgery and he is wrong, he will do far more harm than good. Ethan doesn’t like it, saying Carl will be taken care of medicinally until Ethan knows what to do; but their conversation is interrupted when Sharon informs them all his medical records are being held hostage at the other hospital.

Sharon and Peter Kalmick (Marc Grapey) sit with Carl’s medical team, feeling the bankruptcy of the other hospital is causing mass chaos. MRS has the files and is threatening to destroy them as Ethan and Marcel argue their sides; causing Sharon to wonder what is causing the rift between the two so she asks them both to explain everything on their views.

Natalie tells Lucas’ parents that he is dehydrated and it seems like he has a bacterial infection, she recommends antibiotics but his parents refused, saying Lucas broke out in hives from them. They use natural options before anything nowadays. She suggests a saline IV which they agree to but persists on the antibiotics. Will’s patient is swelling up more and feeling “winded”. He wants to send him for a CT to check out that blood clot. Will speaks to him briefly about his fiance and congratulates him on his upcoming spring wedding.

Sharon speaks to Carl, then returns to the doctors, saying he chose the surgery and Carl Harris is now Marcel’s patient. He tells Noah he will see him in the OR, who smiles, realizing how huge this ordeal is. Dr. Charles spots Eunice at the hospital doors, she admits she cannot control this and needs help. He guides her, saying she can get it and brings her back in.

Will’s patient is worried about how long his test is going to take as he needs to wait for Aubrey. Natalie tells Lucas’ parents he has a fever but now he has pneumonia, but they challenge her because she has been saying it was a sinus infection all day. The parents want to take Lucas home, who are insulted that she is not agreeing with their holistic ways. Will pulls her aside, forcing her to explain the situation. Will reviews the chart, saying he will speak to them alone, upsetting Natalie. They agreed to the chest x-ray but he reminds Natalie that these are the second parents in a month who have lost faith in her so she better be right with this test because it looks like a sinus infection.

Maggie is getting her chemo when CC is walking through the hospital, offering snacks. She has no idea, forcing Maggie to tell her only Sharon knows and she wants to keep it that way; she agrees to ask if she can sit as she has a whole bag hanging and she isn’t going anywhere soon. She’s grateful for the company. Maggie calls them “sisters in arms”, insisting she is managing with the secret, but CC reminds her there is strength in community, but Maggie doesn’t want the pity and complications. CC suggests she let her friends be friends as it can be a real comfort; the two hold hands.

Marcel quizzes Noah who talks out how they are going to do the procedure, Marcel talks to Carl, not sure if the body can hear but hopes the soul can. Noah reviews everything and Marcel is ready to proceed.

Daniel meets CC outside, who claims she is so happy. She suggests making a lasagna for dinner and invites Sharon over, as it is nice to hand out with friends. Maggie is finished her round of chemo when she checks her phone and there is a message from April asking her if she is okay.

Will’s patient, Neil has an embolism that has traveled to his lungs. They are sending him up to IR as Will says he was unable to contact Aubrey as he got distracted. Suddenly his stats go flat and Will attempts to revive him. They work hard on him, as Will shakes his head yelling, calming down enough to announce the time of death. Will is told not to sweat the review as there is nothing he could have done but Neil’s fiance Aubrey has arrived, completely oblivious about what happened.

Noah tells Ethan there were no complications as Marcel was right to book the surgery. Noah feels Ethan should just enjoy the moment as they saved the man’s life. Noah walks away, causing Ethan apologizing, saying had they waited Carl would have died. He wants Noah to succeed, but Noah can promises if he has questions or needs advice he will find Ethan but he has to get back to recovery for when Carl wakes up.

Maggie meets April, Doris, and Paula outside the ED, causing them to ask her what is going on, She removes her wig, making April approach her and comfort her; all four women hold hands.

Daniel returns home as CC says she is in the kitchen. He finds Bert in the kitchen with her. Bert leaves to set the table, giving them a chance to talk. He is not pleased that she is meddling between Sharon and Bert. He insists they cannot spring this on them, who went through a terrible divorce this last year. Suddenly the doorbell rings and Daniel is forced to answer it. She brings a bottle of red and white wine, knowing how CC’s lasagna goes well with both. Sharon spots Bert and both are puzzled as CC asks who is hungry.

Natalie still isn’t sure about what Lucas is diagnosed with as the parents demand to be released from the hospital. Natalie refuses to sign the papers and they want to speak to her boss and their lawyer. Natalie pushes about the medications as pneumonia for toddlers could be fatal. She locks herself in the room with Lucas as her parents scream for security. Security desperately tries to break the door open as Natalie pushes the medications into Lucas’ IV.
